Go to the blog's dashboard and click Ban in the menu. Check the IPs(if you are not sure click Details button and check the Reason) and click the X button.
How to edit the template shown to the banned users?
Search for an online or offline HTML editor(Could try this one), create your page, optionally add the [reason] stamp, then click Source button to get the source code, then copy it in the clipboard.
Go to Dashboard > Ban > Options. And check the textarea for banned by IP, then paste the copied source here.
Then click Save!
If the admin of the blog, bans himself, he will be able to unban? Or to access the dashboard?
Yes! The dashboard and login form won't be affected by the plugin. The administrator can go dirrectly to the login form: www.yourblog.com/wp-login.php , then login, then go to Dashboard > Ban and delete his IP from the banlist.
